i need help filling this out.. please

i have some paper work for my son to starts chool and i need to know what this means it asks for a CYP Representative signature. where it asks for insurance and in the same section its asks for name of insured.. now we have tricare..

plus what is the policy number? is it the sponsors ssn. never had to fill anythign out like this so im confused as heck...

Yes, your Tricare policy number is the sponsor's SSN, and usually Name of Insured is the sponsor's name. If you know your PCMs name you can put it in there if they ask; otherwise i just give the Tricare number and the clinic where we're seen.
