    efmp screening forms

    2792 and 5888 forms correct.. where do I send them after my doctor does the screening?

    im going through this as well right now. trying to get the form from DH and things. im going to ft.drum to have mine done and they told me it needed to be form DA 5888. i dont know if you're moving over seas or whatnot but the other thing i was told was that if it's overseas and military doctor has to do it?

    I called Tri-Care and asked them, and they said my doctor was approved, but the closest base to us is Fort Knox, and thats a good 3-4 hour drive.

    Fort drum is 3 hours and 45 minutes away! so that blows! see i need my passport too and fort drum is the only place around here that does them, well for the military, so i have to go there anyways so might as well get my exam while im there too...thats good though that your doctor is approved it might makes things a tad bit easier id assume.

    My husband is the EMFP Coordinator in his command . He asked "Which branch?"
    Once he has that, he might be able to help.
    (He jsut spent last week at a EFMP Conference )

    We got ours with DH's assignment along with a checklist of everything we needed... He's Air Force.

    I am getting frustrated with trying to figure out what steps to take with this screening. My husband is being no help.
