Moving on my own but still need my tricare?

Ok I have my tricare set up for where I am living right now, however I need to move to another state, but I have a condition that I have to be able to continue going to the doctors about I am fine with changing doctors but my question is how do I go about it if the navy is not moving me out of the state I am in or is there a way to do so?

You call tri-care and tell them you are moving. they tell you what paperwork you need to fill out to change geographical locations and PCMs
