My husband leaves for Navy Basic in a few days. When do I need to get my spouse ID? Do I get info from him on how/when to get it? Where do I need to go? What do I need to take? Thank you!

I *think* that might be an outdated form tho. You can usually call like a local base's directory or something and ask.

Try to do it ASAP. It was a pain in the butt for me to get mine - there was something wrong or missing about DH's information in their database that needed to be fixed. By the time it was fixed he was gone for training and they would not issue me an ID without him being there (as my sponsor). About the ONLY reason I was able to get my ID before he got back was because I knew the Colonel and he was able to pull a couple strings. I had all the paperwork I needed and the information in the database was correct at that time, so it wasn't a big deal.

Normally the way it works (at least in the army) is that they do all there improcessing at bootcamp . You should receive a packet of paperwork in the mail that includes that form signed and filled out by your spouse. You will then take that form to the Id card facility at your closest military installation.
He has to actually have you enrolled in the deers system firat so try not to jump the gun, let all that be done 1st. |

nothing can happen before he leaves- he will need to take any childrens BC, SS cards and marriage license- ALL originals no photo copies!
he will the inprocess - this is when they willinput all the information into the system- get you and any other dependants into DEERS and then will print out filled out forms - he will then send you a packet of information- the app for your ID card and information about tricare and dental. before he leaves make sure you have a POA ( power of attorney) so you can go get your ID card without him.

That's weird - when we tried they made a point to tell us he HAD to get this done BEFORE he left, and if he weren't there I couldn't get my ID.
